cefuroxim iv 1500mg 3dd

General comments

Bij een ernstige CAP (CURB-65 4-5, op zaal) moet altijd een Legionella en Pneumokokken Antigeen test worden verricht: 

  • Indien beide testen negatief: huidige therapie continueren
  • Indien LAG positief: over op monotherapie gericht tegen Legionella. 
  • Indien PAG positief + klinisch verbeterend + geen andere pathogenen: over op monotherapie gericht tegen pneumokokken. 

Bij patienten met CAP, die recent verbleven in landen met een hoge prevalentie voor penicilline-resistente S.pneumoniae (PRSP): overweeg ceftriiaxon iv 2g 1dd 

Bij aangetoonde kolonisatie van de luchtwegen met Pseudomonas spp: piperacilline-tazobactam iv 4000/500 mg 4 dd (indien resistentie voor piperacilline-tazobactam: overleg met de consulent infectieziekten)

Bij fulminante pneumonie in aansluiting op influenza: flucloxacilline iv 6000mg /24u per continu infuus
