General comments
Bij patiënten met een normaal beloop van de klachten is antimicrobiële therapie niet geïndiceerd.
Geef een antibioticum (bij een leeftijd > 3 maanden) bij ernstig ziekzijn.
Overweeg antimicrobiële therapie bij patiënten met een (verhoogd risico op een) afwijkend beloop: bij verminderde afweer, koorts > 5 dagen of opnieuw koorts na een aantal koortsvrije dagen.
Bij contra-indicatie voor doxycycline (zwangerschap, leeftijd < 8 jaar) cotrimoxazol 2 dd 960 mg gedurende 1 week; cotrimoxazol niet bij cumarine, fenytoïne, methotrexaat; in 1e trimester zwangerschap combineren met foliumzuur, in 3e trimester zwangerschap risico hyperbilirubinemie neonaat.
Indien na 2 dagen geen verbetering en een antibioticum wel geïndiceerd is: vervang amoxicilline door doxycycline (of bij contra-indicatie doxycycline door amoxicilline-clavulaanzuur 3 dd 625 mg) of vervang doxycycline door cotrimoxazol.
